Right side axel came out, WHY???

Right side axel came out, WHY???
I have a 1988 F-250 4X4 302CID 8.8 REAR. Apon takeing the cover off
found the retainer c-clip laying on the bottom. clip has markigs that it popped out. Any reason why ??????
I am leary about replacing & driving again
The axel looks fine & gear teeth are good.
