Did it get loose or did it break? Make sure the threads didn't pull out for some reason. If it came loose you probably just need to put some teflon on the treads and tighten it and add oil.

I'd dis-agree with running the same. However you won't need a dramatic change in tire pressure. You increased the width of the tire by 20 mm, therefore increaseing the sidewall(the key factor in psi)...

Reg cab? If your dome light is right next to the 3rd brake light that would be my guess. Other than that I don't see how it could get there without seeing significant damage to your headliner as well.
