Default Clutch

I have a 1993 toyota tocoma and the clutch is slipping. Can this be adjusted? If so how?
Or is a new clutch required. How big of a job is it to replace the clutch?
Also the clutch petal grabs at the top? I'd like to adjust that as well.
Thanks
