It won't fit with the clutch fan and shroud. Unless you have the Power Aid or Air Raid TB spacer. I believe it actually sits in the intake giving you the full inch vs the regular spacers that are 1 inch and sit between the TB and intake. Did that make sence?

so a power aid or air raid spacer will actually work with a manual fan setup?

If I remember correctly it should, but I would do my research 1st. However I would not put a TB spacer on a 5.3. To me it is a waste of time. However if you are interested in 1 I do have 1 only used for a month and taken off. If interested PM me.
