Starting when not plugged in

Default Starting when not plugged in

I have a 2003 F350 with the 6.0 litre.
If I do not plug in the block heater I have to let it warm up for 15 - 20 minutes before it runs smooth and I can drive.
Any suggestions?
I changed the glow plug relay and still have the same problem.

My neighbor must have this problem cause he thinks he has to warm up his Dodge diesel for 30 minutes at 5:00 AM. So does it bother you running rough for a few miles or do you think you are hurting something?
