Chevy 2500

My grandson was in my son in laws 04 chevy 2500 and without keys was able to take the truck out of gear!! No one hurt thankfully! How could that have happened? What failed?

chevrolet express 2500 does not accelerate when warms up.acceletae very slowly ,iBut accelerate on idle , this is a 2000 model and has 220Km on it.can i get any help/ Thanks before
