Default 2002 Chevy Silverado Rear Brake issues/questions

Hello, I have a 2002 Chevy Silverado 4x4 Z71. It fails inspection because the parking brake could not hold properly. So, I thought I have a peek at the inner brake shoes to see how they were looking.

I have everything off, and the drum/disc is loose, i.e., it isn't welded to the hub. But, I cannot get the darn thing off. It comes out, oh, maybe 1/8th of an inch or so, and wobbles all over the place, I figured it would be a cinch, but nooooooo! My e-brake cable is loose, so no brake pressure on it (also confirmed because I can spin the wheels).

So, question 1: what is the trick to get those buggers off? There are no threaded holds in the rotor/disc I can use to get it off.

question 2: another issue is the disc brakes itself. the outside of the disc is all rusted, uneven, etc. indicating to me the pad is not being pressed against the outside of the disc. however, the inside of the disc is smooth, indicating the pad is hitting that when the brakes are applied. The question I have is: what could cause this?

Is there a retaining washer on one of the lug bolts? If so you may have to cut it off with a pair of pliers.
