I have a 06 2500hd 6.0 gas burner 4x4 . I get with less than 10 mpg average with no trailer / no load and less than 6 mpg with a trailer. I have a programmer than didnt help mileage, I am at the point that I must get rid of the truck for the terrible gas mileage, I thought maybe I could have the 4.10 gears changed to 3.08 or so, most of my driving is highway with a few long distance pulls with a trailer. What would you think the mileage would change to after this gear swap, would it be worth it?

Honestly, no it would not be worth it. It would help some, but not as much as I think you are looking for. The fact is, you got a working truck brother. In my opinion, 2500's and up are working trucks and not daily drivers. Another thing to consider is the gear change is going to effect your towing capacity drastically.
Is this truck your primary vehicle? Before I sold the truck, I would consider buying a beater car if you will. Something for say a thousand or two, that will be more fuel efficient and take the load off of driving your HD all the time. In addition, buying a hooptie with a 4 cylinder that gets close to 30mpg would be almost cheaper then the gear change.
Once again, gears would help a little with gas mileage, but remember you havea lot of truck for that 6.0 and there really isn't much that is going to give you extreme gas mileage savings. I wish you the best of luck in your decision.

corvette95 gota agree with SC, changing your gearing wouldnt help your mileage much. I have 3:23's in my truck and ive been asking as many people as i can about the mileage they are getting w/3:73's and 4:10's and the difference isnt more than a couple miles better.
Besides a 3/4 ton truck with 3:08's! I think you'll regret it in the long run.

is there any reason you need a 3/4 ton, or could you get away with a 1/2 ton? if your only doing highway driving id trade in that truck for a nice 1/2 ton that gets better mileage but still able to pull, that is if your not pulling anything major. you should see about a 200% fuel mileage increase too

How heavy is your trailer? A 1/2 ton with 3.73's might be the way to go... At least you won't have to pay the insurance on two vehicles.

I've got an '02 2500HD and I'm getting pretty much 13 mpg city and 14-15 mpg h/w with everything factory except a TBS..... Did you're programmer drop your mileage? I was told that using the programmers could cause you to lose some power and with a heavier truck, that might have dropped you're milage. And what size are you're tires. I'm still running factory specs on mine and they actually look small for the truck but bigger tires will drop you're milage too.
