Aftermarket exhaust install
I recently just installed my Magnaflow dual exhaust on my 99 gmc sierra. I'd say it's very doable as it only took me 2 hours to complete it, however I do all my work at the auto shop on base, so if you don't have all the tools and lift it will probably take longer.
As for performance gains I haven't really noticed a very large difference, but around 4-5k rpm it's noticeable. Whether or not I get there very often, the exhaust looks great and sounds even better. It was worth it for me.

Save the money and buy a magnaflow muffler itself, and have a shop install it for u. I paid 150 for mine total. The stock exhaust is big, and durable for many years. IMO its the same thing as getting a cat back. After installed, give the truck some time to adjust to the new muffler. The computer will tune to it. You will notice it will get louder and perform better.
